後腹膜に発生した成人悪性化奇形腫

A CASE OF RETROPERITONEAL TERATOMA WITH MALIGNANT TRANSFORMATION

　25歳，男性。左上腹部腫瘤を主訴に他院を受診した。左腎門部に発生した後腹膜腫瘍の診断で腫瘍摘出術を施行され，奇形腫と診断された。2年後，左陰嚢内腫瘤を主訴に当院を受診し，左精巣腫瘍の診断にて左精巣摘出術を施行した。病理組織学的診断は精上皮腫であった。術後CT検査，MRI検査にて左腎門部に径2.5cm大の腫瘤を認めたため，化学療法および後腹膜リンパ節郭清術を施行した。病理組織学的診断は悪性化奇形腫であった。

A 25-year old man with retroperitoneal cystic teratoma 2 years previously visited our hospital complaining left testicular mass. High inguinal orchiectomy revealed a pure seminoma. After operation, CT and MRI showed retroperitoneal mass at left renal hilus. He was treated by PE therapy (CDDP and etoposide), but no response was observed. He then underwent retroperitoneal lymph node dissections, and histological findings showed retroperi-toneal teratoma with malignant transformation.
