自動縫合器を用いる膀胱・前立腺摘除術

APPLIED STAPLING TECHNIQUE IN RADICAL CYSTECTOMY AND RADICAL PROSTATECTOMY

　膀胱全摘除術および前立腺全摘除術における膀胱ならびに前立腺摘除過程での自動縫合器利用を試みた。男性37症例のサントリーニ静脈叢処理にMultifire Endo TAを使用したところ，短時間にほぼ完壁な止血処理が可能であった。このうちの18症例では，膀胱側方靱帯と前立腺側方靱帯の止血・切離もMultifire Endo GlAを用いて行ったところ，短時間に出血を認めない側方靱帯の処理が可能であった。女性症例の膀胱全摘除術では膣壁と膀胱側方靱帯の切離に自動縫合器を使用した。

　自動縫合器利用による膀胱・前立腺摘除術は，従来にない泌尿器科開放手術手技の1つになり得ると考える。

We applied the endoscopic intestinal stapler to control the dorsal vein complex in 37 patients who underwent radical cystectomy (32) and radical prostatectmy (5). Stapling the dorsal vein complex was successful in 36 patients. We found this technique using the stapling device achieved excellent hemostasis and less time-consuming compared with the traditional ligating control of the dorsal vein complex. We expanded this stapling technique to ligate and divide the lateral pedicles of the bladder and prostate in 18 patients of these.
