卵管卵巣炎による水腎症の1例

HYDRONEPHROSIS DUE TO SALPINGO-OOPHORITIS: A CASE REPORT

　44歳の女性。子宮内避妊器具を入れていた。発熱，腰痛，下腹部痛にて来院。右水腎を認め，精査にて，右子宮付属器腫瘤による尿管の圧迫が疑われた。尿路閉塞の解除と腫瘤の確定診断のため，右付属器摘除術および右尿管剥離術を施行した。腫瘤の病理診断は卵管卵巣炎であった。尿路閉塞の原因として，婦人科的炎症性疾患によるものも考慮に入れる必要があると思われた。

A 44-year old woman complained of fever, lower abdomi-nal pain and lumbago. She had intrauterine contraceptive device (IUD) inserted for 10 years. CT and MRI showed right hydronephrosis due to adnexal mass obstructing right ureter. The patient underwent resection of the mass (rt. salpingo-oophorectomy) and right ureterolysis. The pathological diagnosis was salpingo-oophoritis. IUD use was thought to be the risk factor of pelvic inflammatory disease (PID). We should remember gynecologic inflam-matory disease in the differential diagnosis of extrinsic ureteral obstruction.
