糖尿病性慢性腎不全患者に合併した陰茎壊死

PENILE NECROSIS IN A DIABETIC PATIENT UNDERGOING MAINTENANCE DIALYSIS

　45歳，男性。糖尿病歴12年，血液透析歴1年。1995年6月13日，亀頭部痛出現。1週後亀頭全体に潰瘍形成外尿道口を中心に灰白色ミイラ状の硬結が触知され，陰茎壊死の診断。抗生剤，プロスタグランジンE₁誘導体投与すれど病変は軽快せず，激痛が持続し，10月16日，陰茎部分切断術施行。術後，疼痛は消失。糖尿病による動脈硬化性血管閉塞が原因で，糖尿病に合併した陰茎壊死は本邦では5例目と思われた。

A 45-year-old patient with diabetes mellitus diagnosed in 1983, and hemodialysis begun in September 1994, was presented because of penile necrosis which began in June 1995. The lesion was resistant to conventional treatments, therefore partial penectomy was performed in October 1995. The patient became free from severe penile pain. Histopathological examination revealed vas-cular obstruction as a result of diabetes mellitus.
