陰茎壊死を来した陰茎絞扼症

Gangrene of the penis due to strangulation : A case report

　40代男性．性交時にプラスチック製のリングを陰茎に装着し，使用後に脱着せず入眠してしまった．その約10時間後にリングは自然脱落したが，陰茎の変色と排尿困難を認め当科受診．陰茎絞扼症により陰茎壊死に至ったため，陰茎部分切除術が施行された．術前の造影MRI所見が陰茎壊死の評価に有用であった．

Abstract

We report a case of strangulation of the penis by a plastic ring. A 40’s man used a plastic ring during sexual intercourse and went to sleep without removing the ring. The ring slipped off spontaneously ten hours later. He visited our hospital for penile discoloration and dysuria. The penis was necrotizing due to strangulation. We performed a partial penectomy. Preoperative MRI imaging with contrast enhancement is useful to evaluate penile necrosis (Rinsho Hinyokika 70 : 183-186, 2016).
