　73歳，女性。頻尿の精査にて超音波検査を施行したところ偶然左後腹膜腫瘤が発見され，精査のため当科へ紹介となった。CT検査と血管造影で左腎上方に腫瘍が認められた。¹³¹Iアドステロール副腎シンチでは患側の取り込み低下，対側の取り込み増加がみられた。内分泌学的検査では異常がなかった。内分泌非活性の副腎癌を疑い，腫瘍摘出術を施行した。正常副腎が腫瘍後面に認められた。摘出した腫瘍は14.5×10×8cm,520gであった。病理診断は平滑筋腫であった。

A 73-year-old woman visited a clinic because of urinary frequency and was incidentally found to have a retroperitoneal mass by ultrasound study. CT scan and selective angiography revealed the mass at the left adrenal region. Adrenal scintigraphy using ¹³¹I-Adosterol demonstrated a marked uptake on right side anda faint uptake on left side. Endocrine function tests were all normal. A thoracoabdominal incision disclosed a normal left adrenal gland at the posterior aspect of the mass. The mass was removed en bloc with the left adrenal gland.
