　65歳，男性。食欲不振，咳嗽，発熱にて入院。超音波検査にて左腎腫瘤を指摘される。画像診断にて膵に浸潤した左腎腫瘍と診断。肋骨下弓状切開にて開腹。左腎腫瘍の膵への直接浸潤を認めたため，腎，副腎，膵尾部，脾，下行結腸の一部を一塊に摘出した。病理診断は膵へ浸潤した腎細胞癌pT4,pN0,pM0であった。術後1年経過したが再発傾向なく通院中である。

A 65-year-old male was admitted to our hospital because of loss of appetite, cough and fever. The left renal mass was disclosed by the ultrasonography. The diagnosis of left renal cell carcinoma with direct invasion to the pancreas tail, was made by CT and MRI. The patient was operated through bilateral subcostal incision and the direct invasion of the tumor to the pancreas was confirmed, therefore the left kidney, the adrenal, the tail of pancreas, the spleen, and the part of the left colon were removed en bloc.
