血球貪食症候群を発症した気腫性腎盂腎炎

INFECTION-ASSOCIATED HEMOPHAGOCYTIC SYNDROME WITH EMPHYSEMATOUS PYELONEPHRITIS: A CASE REPORT

　症例は60歳，男性。食思不振，発熱を主訴に当院を受診した。入院後の検査にて，両側気腫性腎盂腎炎と判明し，治療を行った。経過中，汎血球減少の状態となり，骨髄穿刺にて，成熟した組織球の増加とそれらによる血球貪食を認め，血球貪食症候群と診断した。気腫性腎盂腎炎の治療中に血球貪食症候群を発症した症例は，本邦においては，その報告例は認められなかった。今後，適切な診断法，治療法の確立が重要な課題であると思われた。

A 60-year-old man with diabetes mellitus was admitted to our hospital complaining of high fever and poor appetite.

According to imaging studies, the patient was diagnosed as bilateral emphysematous pyelonephritis. After 4 weeks of therapy, he had pancytopenia and high blood levels of ferritin and LDH. Bone marrow examination revealed a proliferation of histiocytes phagocytosing hemic cells. A diagnosis of infection -associated hemophagocytic syndrome was made. Unfortunately, in spite of antibiotics and corticosterodis, the patient died.
