膀胱の明細胞腺癌の1例

CLEAR CELL ADENOCARCINOMA OF THE BLADDER: A CASE REPORT

　67歳，男性の膀胱より発生した明細胞腺癌の1例を経験した。膀胱に発生する明細胞腺癌はきわめてまれで，これまでに本邦では自験例も含めて7例の報告があるにすぎない。

　病理組織学的所見では腫瘍細胞は鋲釘型細胞と明細胞で構成され，乳頭状の増生を示していた。自験例は腫瘍の発生部位が三角部だけでなく左側壁と後壁にもあったことより，その発生母地はミューラー管由来の可能性が示唆された。

A case of clear cell adenocarcinoma of the bladder in a 67-year-old male is reported. Clear celladenocarcinoma, so-called mesonephric adenocarcinoma, is a rare primary tumor of the bladder, with only7 cases having been reported in Japan including our case.

Histopathological examination of our case showed that it was composed of large cells with clearcytoplasma and hobnail-shaped cells arranged in a papillary pattern. Since the tumors were located in theleft lareral wall and the posterior wall as well as the trigone, it is considered to have originated from theMüllerian duct.
