前立腺肥大症に対するバルーン拡張術の成績

CLINICAL EVALUATION OF BALLOON DILATATION FOR TREATMENT OF BENINGN PROSTATIC HYPERTROPHY

　比較的重篤な基礎疾患を有する前立腺肥大症患者の新治療法として非観血的，低侵襲のバルーン拡張術が行われている．我々はこれまで実施した65例の治療成績を検討した．期間別評価では8ヵ月以降には残尿量の増加傾向がみられた．腺腫の大きさによる最大尿流量率（Qmax値）の比較では小腺腫（30g以下）において有意に改善した．その差は1ヵ月目よりみられ，8ヵ月以降により顕著になった．総合評価では治療後1〜2週間では不変が多いが，1〜2ヵ月で有効が増加し，6ヵ月では著効が増えている．しかし8ヵ月以降には著効の減少がみられた．これらの成績から本法の効果持続期間は1年以内と推測される．

This study evaluates the effectiveness in the production of symptomatic relief and objective improvement by balloon dilatation performed in 65 patients. Residual urine volume once decreased by balloon dilatation, increased again after 8 months. The effects of dilatation on peak uroflow was prominent in patients with smaller glands, showing that the significant differences between smaller and larger glands appeared at 1 month. Overall results disclosed transient improvement during 1-6 months, however, no prolonged improve-ment thereafter.
