In the majority of cases, the varicocele operation ends in failure because of the persistence of the varicocele, and not because of real recurrence. Therefore, preoperative knowledge of the testicular venous drainage routes and the numerous possible venous shunts greatly influences the choice of surgical procedure and help to determine the optimal level for surgical ligation. For this reason we increasingly do selective phlebography of the internal spermatic vein in the primary diagnosis. Most recently, nonsur-gical transcathter occlusion of imcompetent spermatic veins has been described.
