緒言
精索静脈瘤は一般に加療する必要のないことが多いが，次にあげるような適応症には根治的手術が積極的に行われている。1）著しい陰嚢の変形，2）陰嚢部疼痛，不快感，3）静脈瘤が急速に増大する場合，4）症候性静脈瘤，5）造精機能障害が認められる場合，などである。最後の項目については近年，男子不妊症の有力な原因として注目されており，治療後に妊娠成立率が圧倒的に高く，諸家の報告では30〜50％に妊娠の成立をみている1,2）。
ところで精索静脈瘤に対する根治術としては古くはもつぱら怒張している静脈瘤の切除が行われてきたが，この方法では内精巣静脈を通じて，うつ血の原因に対しての処置が講じられていないため，高頻度に再発がみられた。そこで今日では内精巣静脈の高位結紮法が一般化している。通常の静脈分布からみれば高位結紮が内鼠径輪の高さで行われることで十分に治療の目的を達しているが，それでも無効例が0.2〜25％にみられる3）。
In the majority of cases, the varicocele operation ends in failure because of the persistence of the varicocele, and not because of real recurrence. Therefore, preoperative knowledge of the testicular venous drainage routes and the numerous possible venous shunts greatly influences the choice of surgical procedure and help to determine the optimal level for surgical ligation. For this reason we increasingly do selective phlebography of the internal spermatic vein in the primary diagnosis. Most recently, nonsur-gical transcathter occlusion of imcompetent spermatic veins has been described.
