　58歳，女性．約11年前一時的な意識消失後右半身麻痺となる．約1ヵ月前から歩行困難増強し入院．左側被殼出血，両側被殼梗塞，右視床陳旧性出血などがみられるほかに低カリウム血症の持続あり，精査の結果左副腎腫瘍による原発性アルドステロン症の診断で手術．左副腎に13×10×9mmの腺腫が存在した．術後間もなく血圧正常化し，杖歩行が可能となり，2年余を経過した現在高血圧の再発はみられない．

A 58-year-old woman, who had been suffering from right hemiparesis for eleven years was admitted to the internal medicine of our hospital due to severe gait disturbance. Plasma electrolyte estimation revealed a serum potassium value of 2.5 mEq/l. Further examination suggested primary aldosteronism due to left adrenal tumor. An operation and following pathological examination confirmed left adrenocortical adenoma Her blood pressure and other laboratory findings returned to normal within a few weeks after operation.
