　77歳，男性．肉眼的血尿を主訴に来院．膀胱鏡検査にて，前立腺部尿道に尿道粘膜を貫いて出た乳頭状腫瘤を認めたため，尿道腫瘍と診断し，経尿道的切除術を施行した．病理検査で，乳頭状増殖を認める高分化型腺癌の組織像を示し，前立腺特異抗原の免疫染色にて陽性像を認めたため前立腺乳頭状腺癌と診断した．自験例は本邦15例目と考えられ，Diethylstilbestroi diphosphateの内服にて経過観察中であるが，術後9ヵ月の現在，再発，転移を認めていない．

A 77-year old man with the chief complaint of macrohematuria had papillary tumor which penetratedthe urethral mucosa in the prostatic urethra revealed by cystoscopic examination. The tumor was resectedtransurethrally. The histopathological examination showed well differentiated adenocarcinoma with papil-lary growth and PSA (Prostatic Specific Antigen) positive stain immunohistochemically. Therefore, thetumor was diagnosed as prostatic papillary adenocarcinoma. This was the 15th case in Japan. The patient hasbeen treated with diethylstibestrol diphosphate postoperatively, for about 9 months and, he is well withoutmetastasis.
