前立腺末熟奇形腫の1例

A CASE OF IMMATURE TERATOMA OF THE PROSTATE

　16歳，男性。主訴は腰痛と排尿障害。CTにて腫大した前立腺と第1腰椎の転移性病変を認めた。前立腺生検の結果，間葉肉腫と診断されたため，VAC療法を施行したが腫瘍は縮小しなかったため再生検を施行，未熟奇形腫と診断された。腫瘍が化学療法により良性転化を来したものと考え，胚細胞腫瘍に準じたレジメンに変更したところ腫瘍は縮小したが，結局診断から23か月後に死亡した。

The 16 years old boy complaining lumbago and dysuria visited our department. CT scan showed metastatic lesion on the first lumbar vertebra and enlarged prostate. The prostate tumor was histologically diagnosed as mesenchymal malignant tumor and 3 courses of VAC chemotherapy (Vincristin, Actinomycin D, Cyclophos-phamide) were administered. The effect of chemother-apy was not significant, therefore re-biopsy of the pros-tate tissue was performed, which demonstrated extrager-minal immature teratoma with the benign transformation.
