Fournier壊疽の1例

A CASE REPORT OF FOURNIER'S GANGRENE

　71歳，男性．脳梗塞にて脳神経内科に入院，尿道カテーテル留置中であった．陰茎，陰嚢の腫脹を主訴に来院．強力な抗生物質の投与にもかかわらず，炎症が陰嚢内容，前立腺，鼠径部リンパ節まで及んだ．陰嚢内容，陰茎，壊疽部の切除，創傷清掃，排膿，膀胱痩の造設にて良好な経過をえた．長期排尿管理で膀胱痩の重要さを認識させられた症例であった．

A 71-year-old diabetic male visited our hospital with a chief complaint of penis and scrotum swelling.In spite of strong antibiotics administration, the inflamation extended to the penis, urethra, scrotum,epididymis, and prostata. Inguinal lymphnodes were easily palpated. From these findings, the diagnosis ofFournier's gangrene was established. Necrozing tissues was resected and cystostomy was performed. Debrid-gement were tried several times and he recovered. We believe cystostomy is better than indwelling urethralcatherter in long-term treatment of this disease.
