尿道拡張術後に発症したFournier壊疽

A CASE OF FOURNIER'S GANGRENE DEVELOPED AFTER URETHRAL DILATATION USING METAL ROUND

　82歳の男性。前立腺癌にて内分泌療法の加療中，尿道狭窄を来たしたため金属ブジーによる尿道拡張術を施行した。術後7日目に悪臭を伴う陰嚢および会陰部皮膚の壊死と周辺部の皮膚の握雪感を認めたため，Fournier壊疽の診断のもとに入院，ただちに切開排膿と壊死組織の切除を施行した。連日のデブリドマンと創部洗浄および全身的抗生剤投与にて，救命し得た。

A 82 year-old male patient with prostatic cancer under endocrine therapy was admitted because of swollen, grayish scrotum and perineum, presenting sporadic skin necrosis. 7 days before, he was given an urethral dilatation using metal sound for urethral stricture. Immediately after admission under the diagnosis of Fournier gangrene adequate debridement and drainage was done through the scrotal incision as well as systematic administration of broad spectrum antibiotics, and he eventually survived.
