精巣上体吸引精子による体外受精の試み

EPIDIDYMAL SPERM ASPIRATION FOR SEVERE MALE INFERTILITY

　精巣上体管から直接精子を採取しそれを体外受精に供する方法を試みた．対象は閉塞性無精子症，原因不明（精巣上体管閉塞？）の無精子症，化学療法に抵抗する重症膿精液症，脊損による射精障害の計5例である．手術は顕微鏡下に行い，精巣上体管に直接針を穿刺し内容液を吸引する．残念ながら妊娠には至らなかったが，5例中3例で多数の運動精子の採取ができ，23卵中12卵52％で受精が確認された．本法は重症男子不妊症に対する画期的治療法になると考えられた．

Spermatozoa were aspirated from the epididymal ducts of 5 patients who were severe infertile and used for in vitro fertilization. The surgical technique was as follows : Under magnification with an operating microscope, a tiny incision was made with microscissors into the epididymal tunic to expose the tubules. Sperm were aspirated directry from the duct. Many motile sperms from the epididymis were obtained in 3 patients, and following IVF, embryos were observed in 12 eggs (52%). But no pregnancy was obtained.
