腎動静脈瘻を伴った腎血管筋脂肪腫

RENAL ANGIOMYOLIPOMA WITH ARTERIOVENOUS SHUNTING

　47歳，女性。左側腹部痛を主訴に当科受診。超音波検査で左腎腫瘍を疑われ，入院となった。動脈造影像では，新生血管，動脈瘤様の拡張血管を認めた。また，造影剤注入3秒後に腎静脈が造影され，腎動静脈瘻の存在を認めた。病理組織学的に腎血管筋脂肪腫と診断された。

　腎動静脈瘻を伴った腎血管筋脂肪腫症例は，文献上本邦8例目である。

We describe a case of angiomyolipoma with arter-iovenous shunting. A 47-year-old housewife was sud-denly seized with colicky pain in the left flank. She was found to have a renal mass through an ultrasonogram. Selective left renal angiography was performed. An early draining vein just 3 seconds after injection of contrast material was identified. Generally, arteriovenous shunting is not characteristic of angiomyolipoma. This is the eighth case of angiomyolipoma with arteriovenous shunt-ing in Japanese literature.
