後腹膜腔に発育した粘液性嚢腫

A CASE OF RETROPERITONEAL MUCOUS CYST

　74歳，男性。右下腹部腫瘤と右下肢の軽度運動障害を主訴として内科で受診し，エコー，CTなどで，後腹膜嚢腫を疑われたため当科に紹介された。手術を施行したところ，嚢腫は後腹膜を通じ虫垂と連結していたため，嚢腫とともに虫垂も切除した。内容物は淡黄色ゼリー状で720ml,954gであった。病理組織学的所見などから，虫垂部の重複腸管に発生し，後腹膜腔に発育した粘液性嚢腫と考えられた。

A 74-year-old man was admitted to Tokyo Medical College Hospital with complaint of right abdo-minal mass and slight motorial disorder of the right lower limb. An IVP, CT, and ultrasonograhy demonstrated the retroperitoneal cyst. The cyst was removed with the appendix, because they were con-nected tightly with each other through retroperitoneum. The contents of the cyst was yellowish jelly, 720 ml and 954 g. The CEA level of the jelly was 2723 ng/ml, which suggested the origination from digestive tract.
