腎固定術後に発症した肺塞栓症

PULMONARY EMBOLISM AFTER NEPHROPEXY : A CASE REPORT

　19歳，女性．高熱および右側腹部痛を主訴に来院した．腎盂腎炎を伴った右遊走腎による水腎症の診断で腎固定術を施行した．術後10日目に歩行開始直後，失神しショック状態に陥った．肺のシンチグラムとデジタルサブトラクション血管撮影にて肺塞栓症と診断し，血栓溶解および抗凝固療法にて治癒できた．本症が危険因子のほとんどないと思われたケースに発症したので報告した．

A 19-years-old female was admitted to our hospital complaining of high fiver and flank pain. Nephro-pexy was performed for righ renoptosis with hydronephrosis. The patient suddenly had chest pain followed by shock immediately after she began to walk on 10 th postoperative day. Arterial O₂ was 47.2 mmHg in spite of high O₂ inhalation and pulmonary perfusion scan and digital subtraction angiography showed multiple defects of bilateral lung fields. A diagnosis of pulmonary embolism was made. Thrombolysis and anticoagulant therapy was successfully performed.
