幼児巨大水腎症の1例

GIANT HYDRONEPHROSIS IN PRESCHOOL CHILD

　3歳10ヵ月男児。著明な腹部膨隆を主訴として来院した。IVP,CTにて左巨大水腎症と診断し左腎摘出術を施行した。原因は腎盂尿管移行部の線維性狭窄によるもので，内容液は黄色透明で4,200mlと本邦乳幼児報告例中2番目に大きなものであつた。術前みられていた高血圧は術後正常化し，8ヵ月後の現在健康である。

A 3-year-old boy was admitted to the hospital with the chief complaint of extreme swelling of the abdomen. Excretory urogram revealed left non-visualized kidney and CT scan showed a huge water density mass from abdomen to intrapelvic area. Blood pressure was high (160/80 mmHg) but renin-angiotensin examinations revealed unremarkable change. Under the diagnosis of left giant hydronephrosis nephrectomy was performed. The aspirated fluid was yellow clear and 4,200 ml in volume.
