膀胱線維肉腫

A CASE OF FIBROSARCOMA OF THE URINARY BLADDER

　21歳女性。血尿と排尿困難を主訴に受診。膀胱鏡で左膀胱底部に鶏卵大の粘膜下腫瘤を認めた。経尿道的生検で膀胱肉腫と診断され，膀胱全摘除術兼回腸導管造設術を施行した。病理診断は線維肉腫GIIで，術後1年半余を経過した現在も腫瘍の再発転移を認めていない。自験例は文献上，本邦第15例目と思われる。

A 21-year-old female patient was admitted complaining of gross hematuria and dysuria. Cytoscopic examination revealed a hen's egg-sized polypoid mass at the left fundus of the bladder and transure-thral biopsy showed sarcoma. Total cystectomy with ileal conduit formation was performed, because neither infiltration to the adjacent organs nor involvement of the pelvic and para-aortic lymphnodes was recognized. Histopathologically this tumor was grade Ⅱ fibrosarcoma invading only into the su-perficial muscle of the vesical wall. This may be the 15th of such cases reported in Japan. She is well now 19 months after surgery.
