前立腺粘液癌

MUCINOUS CARCINOMA OF THE PROSTATE

　患者は75歳男性。以前より前立腺癌として近医に通院していたが，排尿障害が増悪し当科受診。尿道造影，尿道膀胱鏡，CTなどより前立腺癌の再燃を疑い膀胱前立腺全摘術を施行した。病理診断は粘液癌であり，組織中に前立腺特異抗原の存在を認めた。経過中酸性フォスファターゼは正常レベル以下であり，諸検査で明らかな転移は認めていない。術前診断上CTが比較的有用であつた。

A 75-year-old man was admitted with complaints of dysuria. TUR-P was performed and diagnosis of mucinous carcinoma of the prostate was established. As the tumor became enlarged gradually inspite of the administration of diethylstylbestrol, total cystoprostatectomy was performed. Histopathologi-cally the tumor contained abundant amount of mucin and existence of PSA was confirmed immuno-histochemically. The patient has remained free of disease for 7 months. From the literature our case appears to be the 17th case of mucinous carcinoma of the prostate in Japan.
