前立腺癌を合併した前立腺嚢腫

A CASE OF PROSTATIC CYSTOMA ASSOCIATED WITH PROSTATIC CARCINOMA

　70歳，男性．主訴は尿閉．直腸診では，前立腺右葉から外側にかけて波動性のある大鶏卵大の腫瘤を触知した．画像診断などにより前立腺嚢腫と診断し，嚢腫穿刺吸引および経尿道的嚢腫壁切除術を施行した．内容液は80mlで，血性．嚢腫壁の病理組織診断は低分化腺癌であった．前立腺癌を合併した前立腺嚢腫はわれわれの症例を含め本邦では6例が報告されており，術前診断の困難さを中心に報告する．

The patient was a male aged 70. His chief complaint was urinary retention. By digital examination, a large hen-egg-sized tumor was palpated protruding from the right prostatic lobe to the outer region, and fructuation was felt. Needle aspiration of the cystoma and transurethral excision of the cystoma wall were performed under the diagnosis of prostatic cystoma. The fluid aspirated was 80 ml and was cylologically classified as class II but histopathological diagnosis of the cyst wall all was poorly differentiated adenocar-cinoma.
