襄胞腎の腎外傷の1例

A CASE OF POLYCYSTIC KIDNEY FOUND IN ACCIDENTAL RENAL TRAUMA

　54歳の男性で交通事故による腎外傷にて発見された腎嚢胞の1例を報告した。腎外傷は腎静脈の裂傷および腎実質の軽度裂傷を伴つたものであつた。術前の超音波検査にて嚢胞腎の外傷と診断したが，出血による全身状態の悪化のため開腹後腎摘出術を施行した。腎外傷時の緊急検査における腹部超音波検査の有用性を認識させられた。

A 54-year old man was hospitalized with the diagnosis of renal trauma in a traffic accident. Abdominal ultrasonography showed left polycystic kidney with retroperitoneal hematoma. Emergency operation was performed with the midline incision of the upper abdomen. Massive bleeding was detect-ed from left renal vein which was lacerated about 2cm long. In order to control the massive bleeding, we performed left nephrectomy. Pathological findings were polycystic kidney with the rupture in the upper pole. The usefullness of abdominal ultrasound examination in the diagnosis of renal trauma was empha-sized
