女性ホルモン投与後に乳汁漏出がみられた男性

GALACTORRHEA ASSOCIATED WITH MEDICINAL TREATMENT OF DIETHYLSTILBESTROL DIPHOSPHATE: A CASE REPORT

　61歳，男性．排尿困難があり某医にて前立腺癌の疑いでリン酸ジエチルスチルベストロールによる内分泌療法を8ヵ月間受け，内服を中止したところ6日目より乳汁漏出を認めた．内分泌学的に高プロラクチン血症と高ゴナドトロピン血症ならびに低アンドロゲン状態があり，精巣は著明に萎縮し生検にて原発性性腺機能不全の状態であった．本症例の乳汁漏出は女性ホルモンによる薬剤性高プロラクチン血症によると考えられ，乳房の腫大のみならず乳汁分泌まできたしたのは，おそらく基礎疾患として原発性性腺機能不全があったためと思われる．

A 61-year old man had been taken diethystilbetrol diphosphate for eight months because he had suspected protatic carcinoma. Soon after he stopped taking this drug, he noted galactorrhea. His genitalia were those of a normal adult male, but both testes were extremely atrophic. Endocrinological studies revealed hyperprolactinemia and hypergonadotropic hypogonadism. Galactorrhea and gynecomastia became im-proved eight months after discontinuance of the drug. Galactorrhea was thought to be caused by the drug -induced hyperprolactinemia associated with idiopathic testicular deficiency.
