　1981年以降6例の副腎腫瘍（クッシング症候群3例，原発性アルドステロン症2例，副腎嚢腫1例）に対し，経胸，経横隔膜，腹膜外式副腎摘出術を行つた。本法は片側副腎に対して，従来の術式に比較して小さい皮膚切開で，到達が容易で視野の中央に病変副腎が存在するために手術操作は容易であると思われた。一方欠点としては，両側副腎の観察が不可能であること，開胸に伴う合併症が予測されることが挙げられる。しかし，前者に関しては各種画像診断法の発達により局在診断率が向上していることで，後者については合併症の少ないことから，本術式も優れた方法と思われる。

Since 1981, 6 patients with unilateral adrenal tumor (3 with Cushing's syndrome, 2 with primary aldosteronism, and 1 with adrenal cyst) underwent adrenoectomy by the method of transthoracic trans-diaphragmatic extraperitoneal approach with the resection of 10th. rib. The advantages of this method are simplicity and excellent exposure of adrenal gland with relatively small incision. It is disadvantagous that contralateral adrenal gland can not be accessible through this approach. The technique of this approach was introduced and excellent result was reported.
