下顎転移を起こした胃癌の1症例

A Case of Metastatic Gastric Carcinoma to the Mandible

　はじめに

　口腔腫瘍に占める他臓器からの転移性腫瘍の割合は約1〜4％であるとされている^1,2）。中でも胃癌の口腔内転移は現在まで本邦で約30症例の報告しかない^3）。

　今回，胃癌の口腔内転移を腫瘍切除術によって効果的に制御し得た症例を経験したので，当院での転移性腫瘍の統計とともに報告する。

　A 69-year-old male complained of hemorrhagic swelling of the gingiva. One year before, he underwent subtotal gastrectomy because of gastricadenocarcinoma (stage IIIb). After resection of oral tumor hemimandibulectomy and right upper neck dissection, the patient survived 4 months with better mastication but died of the primary disease.

　Last five years, we experienced two mandibular metastases out of a 140 metastatic gastric adenocarcioma. It is important to examine any suspicious lesions with radiography and biopsy. Palliative surgical resection is advisable to control oral bleeding and to have better mastication.
