腎基質結石と鑑別困難であつた腎乳頭壊死症と思われる1例

A CASE OF PAPILLARY NECROSIS MASQUERADING AS MATRIX CALCULI

緒言

　腎盂内に形成される有機性物質は腎盂像にて陰影欠損として描出される。たとえば，腎盂腫瘍，凝血塊，肉芽性炎症や稀に報告される軟結石あるいは基質結石が鑑別すべき疾患としてあげられる。また，本邦報告例として稀な腎乳頭壊死症でも壊死組織が腎盂内に貯留し陰影欠損像を示す例も認められる。腎盂像読影に際して，これらの陰影欠損と腎盂腎杯の変化から原疾患を的確に診断することは難しい。本症例は両側腎盂内に多発して陰影欠損を認め，さらに腎杯の鈍円化が著明であつた。この所見は基質結石の報告にみる腎盂像に酷似していた。手術により得られた腎盂内容物を分析し，さらに腎盂腎杯のX線所見から腎乳頭壊死と診断した。腎杯の鈍円化に関するX線学的検討，および腎盂内での蛋白塊の形成に係る疾患に関して，本症例との関連において文献的考察を行う。

A 39 year-old woman was admitted with complaints of lumbago and chill.

It was noted that there were unique multilocular defects on excretory pyelogram which had close resemblance to the cases of matrix calculus in the literature.

Specimen obtained at operation were black, completely muddy, which was composed of abundant protein, a fraction of which resembled to the serum protein.

Discussion was made on the difference of roentogen findings between papillary necrosis and matrix calculus, and on the pathogenesis of protein mass produced in renal pelvis.
