A 39 year-old woman was admitted with complaints of lumbago and chill.

It was noted that there were unique multilocular defects on excretory pyelogram which had close resemblance to the cases of matrix calculus in the literature.

Specimen obtained at operation were black, completely muddy, which was composed of abundant protein, a fraction of which resembled to the serum protein.

Discussion was made on the difference of roentogen findings between papillary necrosis and matrix calculus, and on the pathogenesis of protein mass produced in renal pelvis.
