後腹膜黄色肉芽腫および子宮内膜症性卵巣嚢腫に合併した水腎症の1例

A CASE WITH RETROPERITONEAL XANTHOGRANULOMA, ENDOMETRIAL OVARIAN CYST AND HYDRONEPHROSIS

緒言

　黄色肉芽腫の概念は，1935年Oberlingにより最初に提唱され，黄色腫と肉芽腫の組織学的特徴をもつた後腹膜病変として6例が報告された。黄色肉芽腫性反応は，黄色肉芽腫性腎盂腎炎の報告が最多であるが，その他，身体のいずれの部位にも発生し，後復膜には約30例の報告がみられる。われわれは，後腹膜黄色肉芽腫，子宮内膜症性卵巣嚢腫および尿管周囲炎によると思われる右水腎症が合併した1例を経験したので，若干の文献的考察を加えて報告する。

Herein reported a 44-year-old lady, who had retroperitoneal xanthogranuloma, endometrial ovarian cyst and right hydronephrosis due to periurcteritis. The retroperitoneal tumor, which was located in the retroperitoneum between the cervix of the uterus and the rectum, was benign xanthogranuloma histologically with growth of fibroblasts, foam cells and giant cells. Neverthless, it showed highly invasive characteristics, protruding to Douglas' cavity with breakdown of the peritoneum and invading to the anterior surface of the rectum and to the mucosa of the posterior vaginal wall.
