腎癌（腎摘6年後）の膀胱転移症例

A CASE OF BLADDER METASTASIS FROM RENAL CELL CARCINOMA SIX YEARS AFTER NEPHRECTOMY

緒言

　腎癌は臨床的にさほど稀なものではなく，また肺，肝，骨を始め全身各部に遠隔転移巣を来すことはしばしばであるが，膀胱や尿道などの下部尿路に転移することはきわめて稀であるとされている。また腎癌では時に術後長年を経過し始めて遠隔転移が発現すること（いわゆる潜在性遠隔転移）が特徴の1つとされている。今回われわれは腎癌手術6年後に膀胱転移を来した症例を経験したので報告する。

A 57-year old woman underwent the right transperitoneal radical nephrectomy for renal cell car-cinoma 6 years ago. The pathological diagnosis was "clear cell" adenocarcinoma, stage A. She recieved a course of cobalt therapy, 4500 rads and discharged. She did well untill 6 years after nephrectomy when painless gross hematuria occurred. Cystoscopy revealed a dark red mass, like a ground cherry form and little finger's head size, situated immediately above the right ureteral orifice. Transurethralresection of the lesion was performed. Tisse report consisted of metastatic "clear cell" adenocarcinoma.
