子宮全摘後の神経因性膀胱の尿失禁—臨床像と治療法

URINARY INCONTINENCE IN NEUROGENIC BLADDER AFTER RADICAI HYSTERECTOMY:CLINICAL FINDINGS AND TREATMENT

はじめに

　子宮癌に対する広範性子宮全摘後の神経因性膀胱については，すでに婦人科領域で多数報告されているが（三谷，1956,1970^1）；小林，1962，1977^2）），泌尿器科受診例は陳旧例，重症例，尿管膣瘻合併例などが比較的多く含まれているためか，症状も婦人科領域の報告とは若干異なるようである。

　また，報告の主題もほとんど排尿困難についてであり，尿失禁についてはあまりふれられておらず，比較的放置された分野といえそうである（遠藤，1974^3）；河村，1975^4）；新川，1976^5））。

The clinical findings and urodynamic evaluations of urinary incontinence were analyzed in 34 patients with neurogenic bladder after hysterectomy. Urinary incontinence was noted in 18 patients (53%) in which 7 cases were of marked degree.

The neurogenic bladder symptom after hysterectomy is of a special type, which is neither purely defined to be sterss incontinence nor overflow incontinence, but to be a conbined form.

The clinical findings and urodynamic evaluations of urinary incontinence were analyzed in 34 patients with neurogenic bladder after hysterectomy.
