回腸導管とstoma造設の工夫

TOTAL CYSTECTOMY AND ILEAL CONDUIT CONSTRUCTION FOR BLADDER CARCINOMA AND PROSTATIC CARCINOMA WITH BLADDER INFILTRATION

緒言

　Bricker^1）が1950年に回腸導管造設術の検討を発表して以来，この術式は膀胱癌に対する膀胱全摘後の尿路変更術としてよく知られている。日本においても，特にここ数年この術式が広く行なわれるようになつた。術式の変化も多く報告されたが，膀胱全摘，回腸導管造設の手術については術中出血量が多く，術後の尿管回腸吻合部の狭窄，回腸導管のstomaの腹壁内への陥没と狭窄などの問題がまだ残されているといえよう。これらの諸問題を配慮しながら，われわれは1977年より約1年間に20例の手術症例を経験したので，この術式の改良と手術成績を報告したい。

Massive bleeding, postoperative ureteroileostoma stenosis and ileal conduit stoma retraction and stenosis are still posing a knotty problem for the urologic surgeons in performing total cystectomy and ileal conduit construction. With these postoperative complications and the quest for reducing intraopera-tive bleeding in mind, the authors have performed 20 cases of such operation within a period of one year.

For reducing intraoperative bleeding, the anterolateral capsule of the prostate was dissected first through retropubic approach to expose the puboprostatic ligaments and levator ani fascia.
