神経因性膀胱障害に前立腺肥大症を合併した1症例の治療法

AN OBSERVATION CONCERNING THE THERAPEUTIC METHOD OF ONE CASE COMBINED WITH NEUROGENIC BLADDER DYSFUNCTION DUE TO CAISON DISEASE AND BENIGN SLIGHT PROSTATIC ADENOMA

緒言

　近年老人で泌尿器科を訪れる患者の数が増加しており，したがつて老人の身体的な状況を十分把握した上で治療方針を定めねばならない機会が多い。ここに示す症例も，2,3の病院を訪問して，前立腺肥大症という診断のもとに手術をすすめられていた症例であるが，当院での精査の結果，既往の潜凾病による神経因性膀胱障害を合併せる軽度の前立腺肥大症ということで治療法に一考を要した症例であり，その検査成績，治療法などを報告し，更に潜凾病による神経因性膀胱障害や，最近問題の焦点となつている膀胱頸部の交感神経支配と尿道内圧との関係やX線透視下尿道内圧測定法などに関しても言及したいと思う。

Among aged patients, benign prostatic hypertrophy is often combined with neurogenic bladder dysfunction caused by miscellaneous diseases such as diabetes mellitus, spinal cord lesion and cerebrospinal circulatory dysfunction, which need special consideration.

This paper reported a case of neurogenic bladder dysfunction due to caison disease combined with benign prostatic adenoma and discussed the result of urodynamic studies; urethral pressure profile under X-ray television, electromyographic study of striated muscle of external sphincter and the influence of phenoxybenzamine on UPP curve.
