Medullary sponge kidneyに合併したincompleterenal tubular acidosisの1例

A CASE OF INCOMPLETE RENAL TUBULAR ACIDOSIS ASSOCIATED WITH MEDULLARY SPONGE KIDNEY FOLLOWED BY NEPHROCALCINOSIS

緒言

　renal tubular acidosis（以下RTA）は，一般にdistal RTA, proximal RTAとに分類されているが，泌尿器科領域においては結石を形成しやすいdistal RTAが問題となる。最近distal RTAのpreacidotic stage^1）とも言われているincompleteRTAの存在が知られるようになり，欧米では1959年WrongとDavies^2）の報告以来27例が認められ，本邦では1976年平塚ら^3）によつて初めて1例が報告された。incomplete RTAは尿酸性化障害を示すにもかかわらず，代謝性アシドーシスを伴わない点を特徴とするが，distal RTAとは違つてその病態生理の詳細については不明である。

　私たちは最近medullary sponge kidneyに合併したincomplete RTAの珍しい1例を経験したので報告する。

A 38-year-old woman was admitted to the hospital in March, 1976 with the complaint of asym-ptomatic macroscopic hematuria. Plain renal X-ray films and intravenous pyelograms revealed bilateral medullary sponge kidney with multiple calcification at the cystic parts of the medullae. Laboratory examinations disclosed hematuria and urine with pH 6.8 or over but systemic acidosis was not found. The ammonium chloride loading test showed an acidification defect of the urine with induced systemic acidosis, leading to the diagnosis of incomplete renal tubular acidosis of distal type.
