核種併用シンチグラフィーによる後腹膜腫瘤の診断法

THE DIAGNOSIS OF SUPRARENAL MASS LESIONS BY USING RADIONUCLIDES

はじめに

　排泄性腎盂撮影像で片側腎が下方に変位している場合は腎上部の後腹膜腫瘍や肝腫あるいは脾腫などの病変が疑われる。従来はこのような症例の鑑別診断法として血管撮影法，後腹膜送気法，断層撮影などが用いられてきたが近年著しい発展をとげているscintigraphyを応用することも可能である。すなわち2種類の核種を併用して近接臓器の位置関係から種々の病変を診断すること（com-bined scintigraphy）や，コンピューター処理により画像を鮮明化したり，目的とする臓器だけを抽出すること（subtraction scintigraphy）ができる。

　われわれはcombined scintigraphyとsubtractionscintigraphyを組み合わせる方法により後腹膜腫瘍をはじめとする腎上部の病変を鑑別診断することを試み，興味ある知見を得たのでまだ症例数は少ないがその成績を報告し大方の批判を仰ぎたい。

The displacement of kidney demonstrated by excetory urography without any additional sign or symptome is often encountered and presents a diagnostic problem. The displacement of kidney caused by a suprarenal mass such as adrenal tumor must be differentiated from an enlarged liver or spleen.

The purpose of this paper is to make the differential diagnosis of these cases by means of simple and safe methods using radionuclides. Studis were performed by using the combined liver-spleen-kidney scintigraphy and the subsequent subtraction of the kidney scintiphotograph leaving only the liver-spleen scintiphotograph.
