陰茎壊死をきたしたWegener's Granulomatosis

WEGENER'S GRANULOMATOSIS WITH PENILE NECROSIS

緒言

　Wegener's GranulomatosisはF．Wegener（1936^1），1939^2））によつてくわしく報告された1つの独立疾患である。気道とくに上気道の進行性壊死性病変が先行し，ついで全身に撤布性病巣を生じて各臓器の症状を呈し，とくに腎障害を必発して致命的となり，予後不良な疾患である。現在に至るまで病因が確立されないままに，耳鼻咽喉科，内科，眼科，皮膚科，病理など，広い分野で報告されているが，主な病変が他科領域に属するためか，泌尿器科領域において内外ともいまだ報告がない。

　最近，われわれは陰茎壊死をきたしたWegener'sGranulomatosisの症例を経験した。その臨床経過を報告するとともに，腎障害を惹起するがゆえに泌尿器科医として知つておかなければならない疾患であるので，本症の概要を紹介する。

A 43 year old male visited our clinic with the chief complaint of pus-like discharge from the tip of the penis which gradually became necrotic in spite of various chemotherapy. The diagnosis of Wegener's granulomatosis with disseminated lesion was established from the characteristic histopathological findings of inflammatory granuloma with necrotic angitis by biopsy and past history of ulcerative lesion of the nose. Objective and subjective findings were improved by steroid hormon treatment. Urinalysis and renal function tests were within normal limits.
