婦人科的泌尿器疾患症例の検討—第2報 尿管腟瘻症例

はじめに

　婦人科的泌尿器疾患のうち婦人科手術に起因する尿管通過障害例についてみると，第1報で述べた尿管の瘢痕性癒着による狭窄症例^1）は症状が比較的軽度であり，症例によつては無症状のため気づかずに経過することもある。しかし，婦人科手術後に発生する尿管腟瘻は，患者に与える影響も大きく，ときには原婦人科疾患に対する手術効果をも消失させるほどの影響を与え，日常生活にも支障を生ずるので，術後性尿管腟瘻に対する対策は重要なものということができよう。

　われわれは現在までに婦人科手術後の尿管腟瘻症例を14例経験しているので，ここに検討してみた。

14 cases of ureterovaginal fistula were studied. The causative gynecological surgery were extensive total hysterectomy in 5 cases, simple total hysterectomy in 8 cases and supravaginal hysterectomy in one case, showing the high incidence in simple total hysterectomy than radical surgery for uterine cancer.

The main line for treating this condition was to wait expecting spontanous healing, but since there is a limit, ureteral reconstruction before dysfunction of the affected kidney, was recommended.
