泌尿器科領域における緑膿菌感染—その原因と対策

CAUSES AND PREVENTION OF PSEUDOMONAS INFECTION IN UROLOGICAL WARD

緒言

　緑膿菌感染症は漸次増加の傾向にあり，各科において重大な問題となつてきている。検出緑膿菌を菌型分析した報告によれば，長期間低率の発生が持続している場合は種々の菌型が検出されている。一方，短期間に大量発生している場合は同型の菌が高率に発見されており^1），交叉感染の可能性のあることを示している。

　自験の緑膿菌感染例では菌型分析を行なつていないが，発生状況よりみて菌の院内感染であると考えられた。感染経路の分析とそれに基づく対策により感染例が出現しなくなつたことは，この推測の裏づけとなろう。ここにその間の経緯を報告する。

The incidence of pseudomonas infection in three years were 10% among the admitted patients and 0.5% among the outpatients. The occurrence was studied and cross infection was concluded. The cause of contamination were from urinary bottle, pus basin and water hydrant, and the route was suspected via hand to catheter manipulation.

The spreading was prevented by 1) Minimizing the number of catheterization. 2) Bladder irriga-tion under strict sterilization and avoid unnecessary irrigation. 3) Complete sterilization of instruments. 4) Use chlorhexidine solution for sterilization.
