はじめに

　原発性尿管腫瘍に対する手術的療法としては，腎尿管摘除術兼膀胱部分切除術が常識的に行なわれている。しかし腫瘍の悪性度，浸潤度，そして腎機能の状態などにより，保存的手術法を積極的に採用する人々もいる。

　著者の1人土屋は，わが国で初めて2例の原発性尿管腫瘍に対して保存的手術法を行ない，すでに発表した^25,26）。この2例の経過を報告するとともに，最近2例に同様な手術を施行したので，ここに報告し，かつ保存的手術法の意義について検討してみたい。

Total nephroureterectomy including p artial cystectomy has been a routine and most popular treatmentof choice for primary ureteral tumors. However, there are some who recommend conservative surgery for those cases with good renal function and low grade malignancy of the tumor. Tsuchiya, one of the authers, has performed partial ureterectomy with end to end anastomosis of ureterand partial resection of ureter and bladder followed by uretero-neo-cystostomy in each case of ureteral tumor, with no recurrence for 18 and 14 years. Another 2 cases treated by conservative surgery were reported and the proceedure was evaluated.
