緒言

　精嚢腺は骨盤腔底部の深い部位にあつて，しかも小さな臓器であるので，精嚢腺に手術的操作を加えるには，どうしても狭い手術野の下で行なうこととなり，その上直接それ自身を発見するのにしばしば困難を感ずる。したがつて精嚢腺の手術に際しては広い手術野を得るとともに精嚢腺に最短距離で到達でき，しかも周囲との区別のできにくい精嚢腺を容易に発見できるような操作がぜひとも必要となつてくる。

　一般に広い手術野を得るには，腹壁に皮膚切開を加える下腹部正中線部，鼠径部，旁腹直筋部あるいは鼠径・旁腹直筋部などの切開法が適しており，また短距離でこの小臓器に到達するには会陰部，坐骨部あるいは仙骨部などの切開法が適している。さらに，精嚢腺を発見するには精管にそつていくのが最も簡単であるとされている。

A modified method for bilateral spermatovesiculectomy by extravesical and extraperitoneal approach through lower abdominal median incision, was presented. The ampula is approached by releasing the spermatic cord of the same side and the ampula of the other side is released as to release the spermatic cord of that side. Both seminalvesiculae were removed after they were separated from the surrounding tissue. This method was performed on 17 cases and the surgical techniques, indication, operative findings and postoperative course were reported.
