肝合併切除を施行した副腎オンコサイトーマ

A case of adrenocortical oncocytoma who underwent adrenalectomy and partial hepatectomy

症例は54歳男性。人間ドックにて30mm大の右副腎腫瘍を指摘され当科を受診。精査にて非機能性副腎腫瘍と診断。半年後のCTにて増大傾向を認め，腹腔鏡下右副腎摘除術を施行。術中肝臓との間に癒着を認め開腹術へ移行し，肝部分切除術も施行。病理結果は副腎オンコサイトーマであった。副腎オンコサイトーマは稀な腫瘍であり，非機能性副腎偶発腫瘍において腫瘍径が大きく辺縁明瞭な場合，念頭におく必要があると考えられた。

Adrenocortical oncocytoma is extremely rare disease and are usually nonfunctional tumor. A 63-year-old man was admitted to International University of Health and Welfare Hospital under diagnosis of right adrenal tumor. We performed endocrine hormonal evaluation, which revealed non-functional adrenal tumor. 6 months later, CT examination showed a tendency of increased tumor sign and we performed right adrenalectomy to deny malignant tumor. Histological examination revealed highly eosinophilic granular cells arranged in solid patterns showing the diagnosis of adrenocortical oncocytoma.
