75歳，男性。2006年9月，排尿時違和感を自覚し，10月に紹介受診となった。内視鏡にて膀胱右側壁に非乳頭状広基性腫瘍を認めた。画像診断では膀胱壁外への浸潤を疑わせたが，遠隔転移はなかった。TUR-Bt切除片には，低分化尿路上皮癌組織と小細胞癌組織が混在していた。まず化学療法M-VACを1コース施行したところ，PR以上の縮小効果を得たので，2007年1月に膀胱全摘術を施行した。病理組織学的には，腫瘍の変性，壊死が高度でviablle cellは認められなかった。術後さらにM-VACを1コース追加し，現在まで約3年間再発・転移を認めない。

We report a case of primary small cell carcinoma of the urinary bladder,which showed a remarkable response to M-VAC chemotherapy. A75-year-old man with a discomfort sensation at voiding showed a non-papillary bladder tumor in the right wall by cystoscopy. Histopathological finding of the trans urethrally resected specimen revealed a mixture of transitional cell carcinoma,grade 3 and small cell carcinoma. Neoadjuvant chemo-therapy with M-VAC was effective and then total cystectomy was performed. Histopathologically,no viable cancer cells were observed in the extirpated bladder. One course of adjuvant M-VAC was performed after the operation,and three years later,the patient remains disease-free and asymptomatic.
