　60歳，女性。主訴は肉眼的血尿とめまい。膀胱癌T3aNOM1（小脳転移）の診断で入院した。全身化学療法（M-VAC）にて小脳転移巣は縮小し，ガンマナイフで消失した。膀胱全摘除術および回腸導管造設術を行ったところ，腫瘍細胞は認めなかった。初診時に脳転移のみを認めた膀胱癌症例は極めて稀であり，本邦ではこれまでに報告がない。

A 60-year-old woman was admitted to our hospital com-plaining gross hematuria and vertigo. According to clinical and radiological findings, diagnosis of bladder cancer with solitary celebellar metastasis (T3aNOM1) was confirmed. The brain metastasis was reduced to 50% in size by M-VAC chemotherapy and disappeared almost completely after the gamma knife radiosurgery. Furthermore, no malignant cell was found in the radical cystectomy speci-men (pTO) . Solitaly brain metastasis from transitional cell carcinoma is quite rare. This seemed to be the first report in Japan.
