酢酸リュープロレリン皮下注射によると思われる類上皮肉芽腫

A case of cutaneous epithelioid granulomas caused by subcutaneous injection of leuprorelin acetate

　前立腺癌に対するホルモン療法としてluteinizing hormone-releasing hormone（LH-RH）agonistである酢酸リュープロレリン3か月製剤を，上腕皮下に注射していた患者の注射部位に，硬結および無菌性膿瘍が形成された。皮膚生検を施行し，病理組織診にて類上皮肉芽腫と診断した。他剤への変更およびステロイド軟膏外用にて改善した。発赤や硬結などの注射部位障害は3～15％に起こると報告されている。

Induration and sterile abscess occurred at the upper arm hypodermis as a result of repeated injection of the luteinizing hormone-releasing hormone（LH-RH）agonist, leuprorelin acetate, every three months as hormone therapy in a patient with prostate cancer was described. The histopathologic finding of the biopsy specimen was epithelioid granuloma. The patient improved by using steroid ointment and switching to another LH-RH agonist. It has been reported that the incidence of local skin reactions to depot leuprorelin acetate ranged from 3 to 15％.
