腟内異物による膀胱腟瘻

Vesicovaginal fistula associated with vaginal foreign body-A case report

　われわれは腟内異物を核とした結石により巨大な膀胱腟瘻を呈した症例を経験した。約2年前に自慰目的に腟内にスプレー缶を挿入，腟内でキャップがはずれ，放置していたことが判明した。まず，経腟的に結石を除去し，炎症改善後，膀胱腟瘻閉鎖術を施行した。瘻孔は膀胱三角部に存在し，長径約4cmに及んだ。経腹膜操作，および経腟的操作を併用して瘻孔を切除，閉鎖した。術後排尿障害を認めず，一期的に治癒可能であった。

　A case of a 31-year-old unmarried woman with large vesicovaginal fistula caused by a neglected vaginal foreign body is reported. She inserted a hair spray bottle into the vagina for the purpose of masturbation and its cap was left in the vagina for two years because she could not remove it. The cap eroded through the anterior vaginal wall and subsequently formed a large stone. The vaginal foreign body was removed transvaginally and 3months later,combined transabdominal and transvaginal repair of the vesicovaginal fistula was performed successfully.
