外傷性瘢痕による鼻孔狭窄の2例

Two Cases of Iatrogenic Stenosis of the Nostril

　はじめに

　外鼻孔狭窄は，先天性，外傷性，炎症性，腫瘍性などいろいろな原因で生じる。通常，外傷性瘢痕によって生じる外鼻孔の狭窄は，鼻翼の欠損を伴っていることが多い^1,2）。鼻翼欠損を伴わない瘢痕性狭窄は比較的稀であるが，長期間にわたる挿管チューブや経管栄養チューブの挿入留置を原因として起こることが多い^3）。また，外傷性狭窄の再建方法として瘢痕切除・縫合，植皮，粘膜移植，遊離複合組織移植，各種皮弁法などが報告されている^4〜11）。

　今回われわれは，鼻出血に対する止血処置後に外鼻孔に瘢痕を生じ，狭窄をきたした2例を経験した。組織の欠損量に応じて，手術方法を変えることにより良好な結果を得たので，若干の文献的考察を加え報告する。

In this study, we report 2 patients in whom scar of the nostril was developed after hemostatic treatment for nasal hemorrhage, causing procedures for the repair of tissue defect. We also reviewed the literature.
