両側副咽頭間隙に転移をきたした甲状腺乳頭癌の1例

Papillary Thyroid Carcinoma with Bilateral Para-pharyngeal Space Metastasis:A Case Report

　はじめに

　甲状腺乳頭癌の転移様式はリンパ行性転移が主体であり，喉頭前，気管傍，気管前，深頸部リンパ節への転移が多くみられる。しかし，副咽頭間隙に転移をきたした症例の報告は散見されるのみである^1〜7）。

　今回われわれは，甲状腺乳頭癌が両側の副咽頭間隙に転移をきたした症例を経験したので，文献的考察を加えて報告する。

Papillary carcinoma of the thyroid gland often metastasizes to the pretracheal, paratracheal and deep cervical lymph nodes. The metastasis to bilate-ral parapharyngeal spase itself has not been report-ed, although some cases demonstrated hemilateral metastasis.

A 59-year-old female had previously undergone left hemithyroidectomy and left radical neck dissec-tion for the treatment of thyroid carcinoma. Five years after the first operation, two masses were found at bilateral parapharyngeal spaces by MRI. We performed resection of these tumors, and they were diagnosed to be metastasis of thyroid carci-noma.
