鼓膜所見の正常な先天性耳小骨固着症—小児の手術症例の検討

Congenital Ossicular Fixation without Malformation of the External Ear Canal and the Tympanic Membrane in Children

　はじめに

　中耳奇形の1つである外耳道，鼓膜が正常な先天性耳小骨固着症は，聴覚検査法の進歩で幼小児期に発見されることが多くなった。本疾患は手術により聴力の改善が期待できるが，その病変は様々であり，手術も単なる耳小骨連鎖の再建からアブミ骨手術まで様々な手技が必要となる。このため，聴力改善は必ずしも容易ではない。今までこの疾患についていくつかの報告があるが^1〜6），小児の手術症例についての報告は稀である。

　今回，われわれは先天性耳小骨固着症の小児の手術症例について検討したので報告する。

Postoperative hearing gains in 9 ears of 7 childrenwith congenital ossicular fixation unaccompanied with malformation of the external ear canal and tympanic membrane were investigated. These patients underwent ossicular reconstruction or stapes surgery. Of the 9 ears, fixation of the malleus was found in 2, fixation of the incus in 1, fixation of the stapes footplate in 3, incudostapedial disconnec-tion together with fixation of the footplate in 2, and ossified stapedial ligamment in 1. Hearing was improved postoperatively in all cases.
